Notifications tied to transitions

We have several cases where the state would have different notifications when entering (e.g. head's up, you need to look at this) vs exiting (e.g. someone finished reviewing this).

Our primary use case is for steps with multiple possible transitions we would like to program a different notification to a particular user or group of users based on the outcome, e.g. this was approved vs rejected.This would require that notifications be programmed to a specific transitions.
