I agree with the formatting issues (right side of box doesn't have a margin between it and the outside of the box; the action word at the top (e.g., "approve") is lowercase; and after approval, users get a message that they may close the tab. But, I don't agree with the removal of the Leave a comment field. We like it.
Recap on the item#2...what I mean is, to have an option to hide or display the "Leave a comment" field. As we are not using the comment field in all workflow, certain workflow action we don't need the comment field. It causes some confusion to the user when we display the field but it is not in used.

For e.g. when creator submits the document for approval, the approver can either "Approve" or "Reject" the document. When the approver choose "Approve", hide the "Leave a comment box"; when the approver choose "Reject", display the "Leave a comment box".
For the "Approve" action, we include only the documenttitle, documentstate and document:field in the notification email, we did not display the comment, therefore the "Leave a comment box" is not required.
